Layout plan of the factory with machinery placement and power load details
Identity and address proof of the occupier or owner
Original licence number (in case of duplicate request) or plan approval certificate
Affidavit/declaration for lost licence (if duplicate request)
NOC(s) as required for fire, pollution, labour-safety (if applicable)
Details of workforce and installed HP (horse-power) of machinery
